Delete on Upload Bug

With our large fleet of iPads (over 100 of various iPad mini 2,3,4; Air II; Pro), we've noticed a recent increase in some unexplainable behavior. We have all our iPads set to "Delete On Upload", but randomly, some records are staying on some of the iPads. Sometimes they'll stay on the iPad after repeated refreshes, sometimes they'll subsequentially get removed, but the current set of records will remain. Sometimes, some of the records will appear blank with a checkmark, usually the summary bar is complete. The 'Clean Local Database' seems to help, but not always. This is happening on mulitple sets of forms, different levels of iOs updates, iForm versions, etc... Any suggestions?
