ERROR WITH CONDITIONAL VALUE

Hello there,
I have error with this JavaScript ((q3_sent_sample_for_lab_analysis ==1) || (q16_improvemnt_suggestn_for_lsd !="")) on my form. I implemented it on the Conditional Value. [q3_sent_sample_for_lab_analysis] is a Select Data Type while [q16_improvemnt_suggestn_for_lsd] is a Text Data Type. The condition worked perfectly in the first entry/record, but when I start second entry, the element carrying the Conditional Value showed up without obeying the condition. Please, I need somebody's help. Thank you!

  It may be an initialization issue with q16_improvemnt_suggestn_for_lsd. Verify that Dynamic Value for q16_improvemnt_suggestn_for_lsd is set to {""}.
